have you done anything to the transmission?
have you checked the transmission fluid?is it full and clean?
is it a 1/2 ton?with the 4l60e transmission?
from the way you explain,it doesnt sound as if it is the transfer case
to me,it sounds as if the low/reverse clutches are slipping/binding
if you havent already,change the transmission filter
a clogged filter could cause this problem
you need to replace the filter asap,,it could burn out those clutches,then it will loose all gears

the transmission never shifts like into neutral on its own?

when you say it vibrates from a take off,,does the engine rev and not pull the truck?
does it only do this in first gear?

and,this doesnt sound like a bad transfer case
it sounds as if the low/reverse clutches are slipping
the reason it does it the colder it is ,is because the fluid is much thicker when cold
i would think it will soon quit pulling at all
there is nothing more[other than replacing the filter] that you can do for it from the outside
and,you have already tried the filter
so,i would think that the transmission will soon need to be rebuilt or replaced,,sorry
i dont see an easy fix here
