for third party sick pay made by an insurance company, do I the previous employer show this amount in box 1 or just box 13

If the employee paid all the premium payments for this then the amount is not taxable to them now. It would not be entered in Box 1 .

That would be shown in Box 12 with a J: Nontaxable sick pay

No you do not issue 2 W2s for the same individual.
