I received a form from my company. It says Statement of Taxable Income. I cannot tell if the amount listed on this form is already in my W-2. There is a column that says "Total W-2 Income" on the form.

No. The W-2 amount on the form from my company is not equal to box one on the actual W-2. Yes there is an amount on the form, $14,516. Also on the form it states, "restricted / Non-Qualified Transactions. Yes, restricted stock that was released to me but I don't remember this form from previous years.
There are two transactions listed. Here is one.
Grant date: 4/29/13 - Price/Tax Basis $0.000, Trans date 4/29/15, Value Basis ($) 47.22, Shares 153, Total Price ($) 0.00, Total Value 7,224 and W-2 Income ($) 7,224.

Your question mentions an 83(b) election, did you actually make one make one by filing an election statement?

No I did not file an election statement. However, I neglected to tell you an an important piece of information. My company's parent company, who is the holder of restricted stock, sold my business unit. This looks to be a release or execution of those restricted shares as part of the sale. Make sense?
