My employer has started Aflac insurance premiums deductions in 2015. I now owe $825 in federal taxes when in previous years I had a refund How can I make sure they've reported my W2 correctl

Customer: On my 2015 W2, Box 1 amount is $43515 and federal taxes taken out is only 4229. Box 5 is $47444. My 401k is $3930.00. My aflac pre-tax $2,200. It doesn't make sense to me if my aflac premiums are pre-taxed, then why am I owing $800+. I should just as well do the deductions post tax instead. It wasn't a benefit to me to do it pre-tax.
