I am Canadian working in the US on a H-1b visa. I had a few questions surrounding filling out the W-8BEN form.

Questions:
- Part 6, I have a US SSN, do I include this?
- Part 7, do I include my Canadian SIN?
- Part 10, Special Rates and Conditions, is this applicable? Do I need to fill this out?
- Part III, Notional Principal Contracts, is this applicable? Do I need to fill this out?
- Attachments A & B - this does not apply as my permanent residence is in Canada, correct?

Sorry, what about Part III?

Typically, part III is not applicable unless you have income that is NOT effectively connected with trade or business in the US that is from a US payor. In your situation, probably not applicable.
