Basically, you need to file articles of incorporation (assuming you are incorporating which I highly recommend) with the Secretary of State as soon as possible. Next fill out federal form SS-4
to get a federal employer I.D. number (see www.irs
.gov for the form). When this is complete and you are incorporated you should also file form 1023 as soon as possible with the IRS (see www.irs.gov for the form) to obtain your exempt
status from the IRS. Next, you must submit form CFR-1, articles of incorporation, bylaws and the 501(c)(3) determination letter from the IRS to the Ohio Attorney General's office within six months of formation. They will cut you some slack on the determination letter since the IRS is now taking between 4-6 months to issue them. Go to the Ohio AG's website at http://www.ag.state.oh.us/sections/charitable_law/index.htm to get the forms and information.
