Hi Alice. I'm sorry for you loss. I can help here.

...

Since you can't do this onlin you have two other option;

...

  • Calling the main SS number at 1-***-***-**** (TTY 1-***-***-****), Monday through Friday from 7 a.m. to 7 p.m.; or

  • Contacting your local Social Security office.

...

I thing there may also be a form, I'll keep looking, but they may ave changed the process.

...

Also if you use the locator I linked above to find the closest local office, call to make an appointment so that you don't walk in only to have to make an appointment to come back

Expert:  Lane replied 1 year ago.

Sorry, here's the office locator: https://secure.ssa.gov/ICON/main.jsp

I did go to the Forms index... and when clicking on the link for getting a copy of the SSA-1099 the site simply redirects to this page: https://faq.ssa.gov/link/portal/34011/34019/Article/3706/How-can-I-get-a-form-SSA-1099-1042S-Social-Security-Benefit-Statement?URL=/apps6z/i1099/main.html

...

If you want a FULL earnings record you can use this form: https://www.ssa.gov/forms/ssa-711.pdf

...

But it appears that you'll need either to call the primary number or go into the local office
