Where can I mail my Claimant's Appointment of Representative forms.
I tried to file them with the local office, but the clerk did not recognize the form and she would not accept them.

I am sorry that the Clerk did not accept the forms--did not recognize the forms. He or should have. You were at the right place. Your local Social Security Administration office is where you file the forms. You need to go back and this time ask for a Supervisor. Just tell the Clerk that you have Representative forms to file and need to speak with a Supervisor. See page 2 of the Representative Form's instructions:
