http://www.ssa.gov/forms/ssa-7050.pdf is the link to the form used to make that request of the Social Security Administration, SSA-7050. The form requires a fee, and isn't cheap, but this is the only certifiable record that goes back far enough to support your work history. IRS keeps similar records, but they do not provide them back after ten years or so. That is free, but likely won't meet your needs. If you just want annual totals, that information is available for free online at: http://www.ssa.gov/myaccount/. This too likely won't meet your needs. Thanks for asking at Just Answer.
