EVERY TIME THE CLUTCH IS ENGAGED, THE BELT IS THROWN OFF THE PULLEYS. THE ONE IDLER PULLY IS TILTED.

Not sure what year you have as these decks have changed. Check the spindles an make sure there are no bearings going out and pulleys wobbling. Make sure that you have the guards on the outside spindles as these help keep the belt on. Also try starting the PTO with engine at idle and see if it will take off without throwing belt. If it will run at idle then look underneath at belt and see if a pulley is jumping around or running smooth. Also belt has to be good and not rolled on its side or twisted.
