Is this a Power Gear electric slide?

Unfortunately, that would have to come from Forest River and that is just about impossible. Their Customer Service is about the worst in the industry. I can provide a link to the Power Gear trouble shooting info for you. IF your slide isn't moving or making any noise, the most common problem is the breaker in the feed to the system. That is usually a shortstop breaker near the batteries. Slideouts are the hardest to diagnose here due to the great number of variations and styles. I will go ahead and opt out of this so another tech can offer something if they know this model well. http://lci1.com/assets/content/support/manuals/pg-82-S0140-00.pdf

Most of the control modules have a fuse on them as well that blows when the battery voltage is too low. Aways keep the coach plugged in if you can when you operate the slide rooms.
