Short of this being a hardware fault that will need parts replacement, this usually happens when it is only receiving one channel of audio. Are you certain that audio is feeding into both stereo channels?
have you tried taking those exact wiring inputs and outputs and connecting them to a different amp to test it? This will tell you right away whether it's the amp that's the problem or something else.
