I want to know if I buy property from a landowner then they go into a nursing home, can medicare come back and take the land from even though I leaglly bough and pay fir it?

Medicare can't take it so long as it was what is referred to as an "arm's length transaction". That means you bought it for the market value or close to it and that you and the seller don't have any connection other than just a general acquaintance. What they will want to make sure is that this is a real transaction and not something designed to hide property from Medicare.
