Hello

Have you had him evaluated by a speech therapist?

Are you in the US or still in Saudi Arabia?

Thanks for getting back to me.

I am going to refer your question to one of our pediatricians who happens to be in India. I'm sure he will be able to help you, so please be patient. He isn't online right now, but I'm sure he will be here today sometime.

Answer

Kerry has asked me to have a look at this question,
I am a pediatrician based in India.
From what you are describing this is a case of isolated speech delay.
Assuming that the hearing is normal, the most likely cause is a case of familial speech delay, meaning that some member of the family (generally the father or his brothers) may have spoken late and the kid is likely to follow in their foot steps.
Of course things like fluid in the ear needs to be ruled out by an expert ENT evaluation and speech therapy should be planned in any case given his age and lack of present vocabulary,

thank you for your answer but FOR ME AS HIS FATHER NEVER HAD SPEECH PRBLEM AND SO IS MY OTHR son and is there any hope of him speaking

Answer

Hi,
Without actually physically evaluating him, the answer to this question is difficult, but ny professional opinion is that if we do not have any other problems like Autism, developmental delay in other sectors etc. then the chances of him speaking are pretty good.

Hi
'how can Autism be diagnoste
thank you very much

Accepted Answer

That is a tough question to answer,
It requires a skilled professional to diagnose Autism, since there is no simple blood test (or similar) for diagnosis, and this is essentially a behavioural diagnosis.
As a screening tool...

• Play. Children with autism fail to engage in reciprocal play, where there is a back-and-forth between two people (as when rolling a ball) and functional play, where children play with toys such as dolls or cars in the way intended.

• Motor imitation. Children with autism copy others’ motor movements far less than other children.

• Joint attention. Unlike most children, children with autism don’t often use pointing or other techniques to direct another person’s attention.

Here are a few more important screening markers in younger children...
The National Institute of Child Health and Human Development (NICHD) lists five behaviors that signal further evaluation is warranted:
  • Does not babble or coo by 12 months
  • Does not gesture (point, wave, grasp) by 12 months
  • Does not say single words by 16 months
  • Does not say two-word phrases on his or her own by 24 months
  • Has any loss of any language or social skill at any age.

All these kids will need to see a pediatrician skilled in neurodevelopment, or an allied specialist, who will be able to provide a diagnosis,

Dr. Gupta
