A fishy vaginal odor sounds like it could be Bacterial Vaginosis (BV) happens when the normal bacteria of the vagina are replaced by different ones. This can happen for several reasons, including douching, a new sex partner, and possibly by using an IUD. It is not per say an STD though it can be associated with sex. People who arent sexually active can get BV.
BV at times can go away by itself. But it is always wise to have it looked at, as I dont know writing here if it IS BV, it could be something else. And, there is always the potential that the bacteria that cause BV can affect the uterus and fallopian tubes (a condition called PID). No one knows why, but if you have BV, you are more at risk of getting HIV if you are exposed to it.
My advice: be seen by your GYN, BV is easily treated and its advisable to do so. The Miconazole you are using wont help BV, that is for a yeast infection, and yeast infections generally have thick whitish clumpy discharges and smell yeasty.
