Question

2001 Winstar check engine light is lit with a P-401 code or insufficient exhaust gas recirulation. What could be the problem? I have replaced the oxygen sensors and the egr valve , cleared the code and it keeps coming back on.

There are a couple of possible causes here. Most of the time when this code is set, it's cause by the DPFE (differential Pressure Feedback EGR) sensor. They are pretty notorious for setting this code.

On this particular engine, there is another cause also. The EGR system consists of 6 individual passages that feed to under the upper intake and they can get plugged up with carbon. You can test the DPFE if you have a DVOM and a Vacuum pump.

Actually, there is a recall on this component for your vehicle.
