This could be from a fungal infection, sun damage, a tumor, or an autoimmune diseas such as pemphigus or lupus.
For home care you can see if some neosporin or triple antibiotic ointment or plain petroleum jelly will help with this.
But you should see your vet to investigate this as if its an autoimmune problem you will need other meds such as steroids to help resolve it.
