Trimming the grapefruit tree back will not be helpful in increasing the fruit size or increasing it's fruit production. Using a high quality, slow release citrus fertilizers will help. Pinching off the smaller fruit will increase the size of the remaining fruit but with less fruit than normal that might not be an option. Stress can often decrease fruit size. Keep it properly watered, fertilize and provide proper sunlight and your tree should be back to producing many large grapefruits again. Weather can also play a very good role in the tree's fruit and most of the US has had less than normal weather this year.
