how do i address a package that is being shipped to manila philippines

This is what I have for the address, but in what format do I put it on the package. Also, is there a zip code?

Yes, there should be a zipcode. For the Philippines it is 4 digits and goes before "Manila" though, not at the end.

Well, I know it starts with "14" but I can't tell what the last two are based on the lists I've found. Here are two:
