How do I file a complaint against a dentist in Illinois

I was treated very badly at my oral surgeon's office yesterday. I lost my dignity. My oral surgeon was more concerned with holding her baby than helping me. I need to file a complaint.

The Illinois State Dental Society recommends that you talk with your dentist about the problem first. If that doesn't work, or isn't an option, you can complete a "Request for Review of Dental Services Form" and submit it to the Illinois State Dental Society.

Reply to Kim's Post: No, this form does not help me. It is to report a "clinical" issue with a dentist. This issue was with my oral surgeon who I don't usually go to. I was referred to them by my dentist because I needed to have a tooth pulled and I was in a lot of pain. They totally ignored me for three hours even though I was crying and in pain not one person came out to see me for three hours. It was horrible. I need to find out where to file some kind of complaint, if there is any because the oral surgen was greeting people with her baby on her hip while I was sitting there in the waiting room sick as a dog. The receiptionist were also holding the baby and totally irnoring me. I am so upset. Please help.
