my wife just woke up with a severe headache that's not subsiding with aspirin/ibuprofen
how should we approach this situation?

First of all, it may take 15 min to 1 hour for the medication to work. In the mean time you can try to get her to relax. Perhaps a hot bath/shower. Giver her message. Dim the lights, allow it be quiet. She can try Tylenol in about 4-5 hours. If it's that severe, it may be a migraine... so if there is no change after doing some of the above, you can take her to the ER and they may be able to help her further there.

Reply to Lindie's Post: she said she had a migraine years ago that she felt in the front of her head. This headached woke her up out of a sound sleep and is at the top of her head going down to both ears. and is excruciating. she dosent think this is a migraine.

She knows her body best. If this is something that doesn't seem or feel right, the only option Hun is to have her seen. It may be something simple to perhaps something not so simple. But without her being seen it's just a lot of guessing. So go with her gut instinct if this is something she needs to be checked out. If it's related to sleeping in a wrong position etc.. generally there would be some tension or pain in the neck area too, which you don't mention anything like that. So if the medication she has tried, if there is no change, then you may just want to be on the safe side for now and have her checked out..
