Question

3 weeks ago I underwent an operation to correct an incisional hernia. I had had an operation in 2008 to repair a hiatus hernia. The operation seemed to go fine and the doctors seemed happy with everything. However, 3 weeks on and I'm still draining fluid. I was discharged with a tube drain in but this was taken out as it had come loose (stitching). I've been left with just the hole in my abdomen where fluid drains into a bag. The ammounts have varied from 350ml one day over the last weekend to amounts of about 100ml in 12-15 hours. I'm just not sure this is normal or not - being as it's 3 weeks since the operation and they were talking in the hospital that it would reduce enough that I could have the drain out whilst I was still in - obviously this didn't happen. regards Andy

Info Request

Greetings.

Has a mesh been used for the incisional hernia repair?

Was an ultrasound done post operatively to assess this fluid drainage?

Is the fluid decreasing in amount or is same?

Yes, mesh was used in the repair - from what I can gather, they placed mesh all the way down the orignial incision - the incision is approximateyl 11 inches long, down from the bottom of the rib cage.

Accepted Answer

Hello,

In an 11 inches incisional hernia mesh repair there is an extensive dissection of the layers between fat and fascia (on which the mesh is anchored). The fat looses its blood supply due to the dissection and converts in fluid for considerable time after the surgery (up to 3-4 weeks). So this is likely to stop in coming one week or so. An ultrasound is usually done to just rule out any pocket, specifically made from which fluid might be draining. If there is no such pocket or specific point of collection, this is likely to stop on its own in coming 7-10 days.

Please feel free for your follow up questions.

Ok thanks, no ultrasound was done. Fluid levels seem to fluctuate each day - I've kept my own chart - they range from 40ml - 100+ml. Colour wise, currently it's a pinky brown colour. The large, 350ml amount one day over the weekend was darker in colour.

Answer

Hello,

Ask your physician to send it for culture. I would not be worried if it is 50 ml or less and gradually decreasing in amount. A non infected fluid will be thin, serous and pink or light pink. Any other change in fluid in amount or appearance will require a culture and ultrasound.

It is pleasure and privilege assisting you.

Best wishes and kindest regards,

Dr. Arun
