Question

Hi, My son is 7 months old, diagnosed dairy allergy (although blood tests show no allergy) on a dairy free diet. Last few weeks dark circes around both eyes, stuffy itchy nose and this week red sores all around mouth & chin. Also whenever rests face on his hand, even for a short while, he has his fingers imprinted bright red on his face taking hours to settle. Last few days bright red inflammed area around anus with raw and weeping skin. I would be most grateful if you would advise on what you think is going on. Facial rash? Inflammation and eroded skin on bottom? Dark circles around eyes? Do i need to do anything about it? investigations? anti-histamines? What about the dermatographia, is that associated with anything worrying? Thanks Maria

He has gained weight well and is on the 50th centile. His development is great, crawling and pulling himself to standing, He is very interactive and generally very happy but has become quite miserable since the facial and especially rectal rash. He wkes at night 2-3x and screams inconsolably.

He had colic from birth to 4 months, followed by reflux/oesophagitis from 4 weeks which was later put down to dairy intolerance (vomiting 15x/day). This has settled on medication (ranitidine anddomperidone) and a dairy free diet. He has always had a little inflammation around the anus and have always used sudocream (assumed gastrointestinal inflammation). He is currently teething and and also dribbling a lot, distressed by his teeth and biting a lot.

He also has bilateral hydroceles of the testes (born at term plus 5 days and IVF pregnancy with complications.

I treated th rectal rash as simple nappy rash initially but it worsened and i assumed it may be infected and added 1% cltrimazole which seems to have improved it somewhat.

Thank you for the further information. I really appreciate the level of detail you have provided.

With regards XX XXXX question, this does not strike me as a "red flag" for any portenitally dangerous allergic problem. Initially I was worried about something called acrodermatitis enteropathica which is a zinc deficiency that manifests itself as severe rash around the mouth and anus (these kids are not happy and do not grow well).

It is certainly OK to try an antihistamine to see if that helps. You may want to ask your doctor about checking a blood count to look for a type of cells called eosinophils and an IgE level (these are markers for significant allergic disease). Since there may be some degree of bacterial overgrowth, you may want to try "probiotics" (healthy bacteria)--I use Lactobacillus GG (the American names of the brands I use are Lactinex and Culturel but I get the sense you are not American.

Finally if the rash does not get better you may want to ask that it be tested for strep. This is not very likely at your son's age but something to think about if it persists (this should not respond to antifungals however). A bright red diaper rash is often fungal so this does really seem what you are dealing with.

Thank you for your reply. We have already tested eosinophil levels and they came back normal. What would you suggest regarding bruising around eyes, is this due to allergy/ inflammed sinuses and does it need investigating or treating? Is the lactobacilus dairy free? Is there anything I can use on the facial rash ? Do you think it maybe just from teething and drooling? The only unusual food he ate those days was strawberries and kiwi - are these foods allergenic and should they be avoided until an older age? Thank maria

Thank you for the further questions. The darkening around the eyes is likely due to "allergic shiners" which is usually benign If it is really bruising (dark purple), however, please have him seen right away.

Regarding the probiotics, most do have very small amounts of dairy but there may be dairy-free forms you can find. Please ask your pharmacist abou tthis.

Regarding the facial rash, I would avoid using anything for the time being. Facial skin is sensitive and this is why we tend to avoid topical steroids on the face.

Regarding the strawberries and kiwi, well actually both of these are fairly allergenic so I would avoid them for the time being. In fact, I would start a new food one at a time gradually and avoid all berries, citrus, nuts, legumes for the time being.
