Question

As a registered nurse, what would be the best way to express my opinion that a discharge plan for one of my psych patients is unsafe? I am concerned about a bad outcome, and frankly am worried about future lawsuits. What is my accountability?

My best advice to you would be DOCUMENT, DOCUMENT, DOCUMENT. I would express loudly your opposition to the discharging Dr. that you feel this way and you can also try to set up a safety plan with he patients family if that is an option. If you document that you see warning signs that make him unsafe and that you expressed these concerns to the discharging Dr. or therapist then you will be covered. The Dr. or Therapist will be accountable.
