Question

I take 2-4 tramadol 50mg tablets about 5x a week. I never take more than 2 at a time and never more than 4 a day. I have chronic back pain but to be honest the effects of the tramadol make me a much more pleasant and happy person. Question is whether there are long term effects from my dosage and whether there is an alternative to taking the tramadol...maybe and SSRI? Please answer with as much detail as possible.

Hello, XXXXXXX, I will do my best to answer your question. Tramadol (Ultram) binds to mu-opoid receptors in the brain, but like some SSRI's, it inhibits reuptake of serotonin and norepinephrine in the central nervous system. If the Tramadol is helping your pain, it may be that your doctor could prescribe an SSRI (cymbalta) which also affects the reuptake of serotonin and norepinephrine. The doctor's decision would also include other disorders you may have, other medications you are taking, why you are taking the tramadol, and so forth. It may be that if the doctor decided to prescribe an SSRI, the dose of tramadol may need to be reduced to avoid causing other complications like serotonin syndrome. It would be worth talking to your doctor about it.

Well I think the Tramadol is helping both my pain and my mood. Basically here is the scenario: Is it dangerous to continue taking the tramadol as both a chronic pain reliever and SSRI for very long periods of time? How long could I continue this dose before it causes permanent damage to my liver, kidneys, or related organs? I currently take 1mg propecia daily, and also take milk thistle, a multi vitamin, Omega 3, 6 and 9, and Red Yeast Rice extract. I do not drink alcohol much (maybe once a week) and don't smoke. I also exercise 6 days a week. My central question is for how long I can continue the dose (2-4 50mg tramadol pills daily...or a total dose of 100-200mg daily) before I cause permanent damage? Or, is it safe to continue the dose indefinitely...even considering the supplements I also take? Please provide a thorough response. Thanks in advance. Dave

Depending on your kidney function and liver function tests could help determine the length of time before this dose would damage your vital organs as you mentioned. With tramadol, there is the issue of long term use and physical as well as psychological dependency issues developing. Many doctors will only prescribe it for a short period of time. Also one of the side effects of tramadol is urinary retention and/or frequency. I noticed you take propecia. Is this for BPH or alopecia?

I had blood work done a month ago showing my kidney and liver function was good. Everything was in normal range. I try and take the tramadol for a few days, and then take 2 days off to stave off dependency. I take the propecia for hair loss.

I also noticed you take milk thistle. Is this to treat something specific or as a cleansing agent for your system?

Just a cleansing agent since I am taking the tramadol and the propecia. I only take the propecia about 4-5 times a week. Not every day.

Thank you for the information. It does help.

Since you are not taking it every day and the dose you do take is considered within the normal limits for daily use of the tramadol, as well as the other supplements you are taking, it may take a while before that medication would cause damage to your kidneys, liver, and such unless you started to develop liver and kidney issues. I can't really give you a set time line since each person's body metabolizes meds a bit differently and we don't know what may come in the future that could affect your body. Since it sounds like the affect on your mood is a positive and you don't take the tramadol every day, I think it would be worth it to talk to your doctor about an SSRI to help your mood on a daily basis so on days that you do not need the pain control, you can still have a positive mood which would enhance your quality of life since individuals with chronic pain issues can get depressed at times which can also make the pain seem worse.

OK thanks I appreciate your help. One last question. Would an SSRI give me the positive mood effects of the tramadol but be safer on my liver and kidneys and related organs? My basic goal is to keep me feeling happy and better about life, which the tramadol does provide me, but I'm concerned about long term effects of taking the pills 5-6 days a week. If an SSRI could give me the "happy about life" effects that I get from the tramadol, but with less danger to my system, then that would be a great thing for me. Is that the case? Or would an SSRI only be safe for a short period of time, like the tramadol?

With the SSRI, there is usually not the dependency issue like for the tramadol which is a positive. The dose would depend on your moods and how effective the lowest dose would work for you. With some SSRI, you only take it once a day. I know people who have taken SSRI's since they first come on the market back in the late 80's / early 90's or so and they do not have any long term effects that have been detrimental to their bodies.
