Tramadol (Ultram) binds to mu-opoid receptors in the brain, but like some SSRI's, it inhibits reuptake of serotonin and norepinephrine in the central nervous system. If the Tramadol is helping your pain, it may be that your doctor could prescribe an SSRI (cymbalta) which also affects the reuptake of serotonin and norepinephrine. The doctor's decision would also include other disorders you may have, other medications you are taking, why you are taking the tramadol, and so forth. It may be that if the doctor decided to prescribe an SSRI, the dose of tramadol may need to be reduced to avoid causing other complications like serotonin syndrome. It would be worth talking to your doctor about it.

Since you are not taking it every day and the dose you do take is considered within the normal limits for daily use of the tramadol, as well as the other supplements you are taking, it may take a while before that medication would cause damage to your kidneys, liver, and such unless you started to develop liver and kidney issues. I can't really give you a set time line since each person's body metabolizes meds a bit differently and we don't know what may come in the future that could affect your body. Since it sounds like the affect on your mood is a positive and you don't take the tramadol every day, I think it would be worth it to talk to your doctor about an SSRI to help your mood on a daily basis so on days that you do not need the pain control, you can still have a positive mood which would enhance your quality of life since individuals with chronic pain issues can get depressed at times which can also make the pain seem worse.
