My five month old puppy just had his bloodwork done: The results are: BUN - 35 mg/dL; Creatinine - 1.7 mg/dL; Alkaline Phosph - 149 U/L; ALT (SGPT) - 313 U/L; Platelet Count - 512.010^3; Neutrophilis - 49%; Lymphocytes - 37.0%; Absolute Lymphocytes - 5217.0. No bile tests have been done. Why and what now?

Just labs so far. Trying to determine best use of my money. Vet reccomends ultrasound and opines liver shunt (it is a Maltese), but I was wondering if some of the other tests would be more precise or if other tests are warranted.

He was just going for a vaccination and routine bloodwork. The only symptom I can see are his weight (he is now a whopping 2.2 pounds). He is pretty mellow, with the exception of a few hours each day when he is wide open. We spend 24 hours a day together (he hangs out in my office) and he pretty much sleeps unless he is stimulated. Other than that, I have noticed that, although he wakes up instantly, it takes him a few minutes to get going after a nap. If I just take him outside immediately, he will pee, but the kind of stands there for about 15-20 seconds before shaking and moving around. In the morning, he hangs out in his crate a minute before coming out (but, heck, so do I).

XXX XXX and the lymphocytes are the only values that I'm concerned with--everything else looks pretty normal to me. ALT can be high due to a liver shunt (so bile acids would be worthwhile) or if the pups has exposure to any toxins (any plants he likes to chew on? Have you given him any medications?). Intestinal worms can also affect the liver, but if he has been dewormed 3 times or so, he should be fine there.
The lymphocytes can be high in puppies just because they are still developing antibodies from vaccinations and such.
I guess I would look around in the environment for anything your pup could be getting into and recheck the liver and CBC values in a week or two. If the ALT continues to be high or is increasing, then I would do a bile acids test to check liver function.
