Question

do potential jurors have to disclose personal information (address, phone number, birthday, spouse's name, occupation) in federal court during voir dire or on the questionnaire? What are the consequences for a juror if he refuses to take the oath or answer those questions in a voir dire open to others.

It is available to the clerk of court. This information is not appropriate for voir dire but in the initial questionaire it is provided primarily to make sure you get credit with responding, appearing and for some, paid.

I have been asked to verbally verify the answers to these questions in voir dire before and in front of the the defendent (yeah! I'll vote to convict him now that he practically has directions to my house). I have no intention of doing that again. I don't care if I'm fined for it and I don't care if I'm paid. I also don't trust the court not t reveal that information if I'm empaneled. I realize you're not giving legal advice. I'm just trying to assess my probability of being held in comptempt and thrown in jail for refusing to answer those kinds of questions (I have no problem answering others like whether or not I've been a party in lawsuit, arrested or convicted, ethnicity, religion, etc.).

You should not have been asked to do so and in the future you can ask to speak to the judge to voice your concern over the improper questions.
