She would need to address this problem with the head of that insurance company. She should present documentation from the bank showing fault and should also request copies of her paperwork to see who made the mistake with regards to the routing number. If the issue is not resolved, she can file suit against the insurance company and let the court decide responsibility. She would need to show that she provided all accurate information and either the bank or the insurance agent made the mistake and that she should not be without coverage due to their mistake.
