Your concern is very valid. Being that you have had a PE in the past you have a high chance of this happening again. BUT there are different things the doctors can do to try to keep this from happening. During surgery they would likely stop the coumadin and start heparin until they feel you are ready to go back on coumadin. Also they can use compression stockings, and sequential compression devices.

Before you have the surgery it would be a good idea to see a hematologist and see if there is a reason that you to clot (ie protein c or s deficiency etc). If there is then it would be easier to control as well.

It sounds to me like you will need the operation at some point - but what you should do is push it off as long as possible. (Of course you should get a second opinion as well).
