Did you take temperature on skin or with an anal thermometer?
How long ago was he given Ibuprofen?
Can you take his pulse or heart rate?

Sometimes sweating due to fever or due to medicine gives erroneous reading and temperature comes low. So clean the ear with a dry cloth and again take a reading. If the pulse is more than 100-110 and he his refusing meals with high skin temperature, it would be prudent to go to ER for an evaluation.
If he was covered by thick clothing or blankets, that can raise the skin / body temperature.
Following measures are helpful to treat high grade fever in a three year old;
1) Medications; Ibuprofen (Motrin) or Acetaminophen (Tylenol)
2) Sponge bath with luke warm water
3) Light clothing and light weight sheet to cover the child; overdressing can raise the fever by preventing heat to escape.
4) Plenty of fluid prevent dehydration
