Question

I have had chills, fatigue, headache, body aches off and on for more than a week. I went to the doctor on Friday. My doctor was not there and someone was fillingin for her. I told him I was having pain while breathing. I have had asthma for 16 years, but it is controlled, and I only usually use my inhaler once every couple of months. This summer I had more difficulty than normal with my asthma. I also had a UTI. The doctor prescribed meds for the UTI but basically just told me to use my flow meter and chart the results. But Sunday morning I was in the ER and was diagnosed with pneumonia. I've taken three doses of my antibiotic and two doses of prednisone. Tonight I had a follow up visit with my regular doctor. I was wheezing bad last night and this morning but seem to be breathing a bit better now with less wheezing. However, I am much more weak today than I have been. My doctor tells me that she cannot rule out flu or H1N1. She says to let her know if I start gasping for breath or my fever spikes. The hospital did not test me for H1N1 and my doctor "does not" test for H1N1. I asked her how long I have to worry that I might "take a sudden turn for the worse", and she didn't really give me an answer. She said it's a luck of the draw. She also does not prescribe Tamiflu, because she says it can make symptoms worse. My question is: How long do I have to feel scared to death that I might suddenly take a turn for the worse and end up one of the fatal statistics due to the pneumonia and asthma? I would point out that I have been told it will be at least a month before even the possibility of having the vaccine in my area, so I have not had an opportunity to get the vaccine.

Hi,

Thanks for all the information.

My commiseration with your travails.

You have been on antibiotics & steroids, but not getting better.

Is it a swine flu?

we cannot say without testing.

But, one thing likely, that you may have a viral infection.

Now, there are many respiratory viruses which after causing trouble for a wk or two will subside ( your body will fight it off).

But, some nasty virus( like regular flu virus) , besides swine flu, may also cause severe symptoms.

Given your h/o asthma, it will be wise to start you on tamiflu ( no it does not make your symptoms worse) as well, to cover for viral infections.

If your doctor is not ready yet, you can get a prescription from your local urgent care.

There is no time limit, when this can turn fatal.

In presence of asthma, the answer is anytime!

Infections can cause excessive bronchial hyperreactivity & hence acute respiratory failure.

Hence, please continue albuterol nebulizations, take steroids, rest, adequate hydration, antibiotics & tamiflu as well for now.

Why would my emergency room doctor not prescribed that or even suggested it when I was in there?

Hard to say for me.

ER doctors are very busy & may not know your clinical condition well.

Besides, your symptoms were just started.

But now in spite of antibiotics, with no improvement, the possibility of flu cannot be ignored anymore.

When you are not sure about diagnosis & your condition is not better clinically, it is better to be safe than sorry ( that is, start treatment, then look for diagnosis).
