Can you tell me how he has been acting, is he having problems breathing?
What has your vet recommended?
Is he still at the vet?
How long has it been since he has eaten?

My heart goes out to you. I am worried that his decrease in food and water over the last week is only going to make him worse.
I think that a decision has to be made fairly quickly. If taking him to a specialist is an option for you then I would make an appointment with them right away. If there is a chance that some thing can be done for him the sooner this happens the better.
If you call them in the morning and inform them of his condition they may get you in the same day.
Not eating and drinking can lead to dehydration, and organ failure.
The specialist may be able to hospitalize him to keep him hydrated and make sure he eats. They will also have the tools to better access his condition.
I think having him seen by the specialist will allow you to make a clear and informed decision on where to go from here.
