Question

Hi: I have an 5 months American Bulldog, 3 weeks ago I took him to the vet and he did a scraping and told me it was demo mange, he told me about Amitraz, which I told him to wait (I've read to many bad things) and told me of Ivomec, I went home and gave him the dose of Ivomec and have been bathing him with Dermabens Shampoo and then Goodwinoil. I am also giving him Vitamins. Its been 3 weeks know and he has bumps in neck, shoulder, back and legs. He is smelly, but not that itchy, he only has like 2 or 3 small spots with no hair. What should I do? give more ivomec, bath him more frequently? What do you think of PestBest RX products for mange treatment? Please any help will do great.

Hi,
The Goodwinol cream is fine to use if there are only a couple, localized spots of Demodex. It can take up to 8+ weeks to clear up.

But it sounds like your dog is having a more generalized infection with a secondary bacterial skin infection on top of it. The bumps may be scabs or pustules from bacteria and you will need oral antibiotics to help clear that up.

Ivermectin daily is a good choice for demodex, but it will take up to 3+ months to get it cleared up. You should make sure you are giving the right dose every day and go in for a skin scraping every month to check your progress. You want to continue giving the ivermectin daily until there are 3 negative skin scrapings each a month apart.

I wouldn't do the sulfur cream or the Mitactin spray (is that for ringworm fungus?) as neither will do anything for demodex. Bathing is fine and I would use something like Malaseb, that will help control both bacterial and fungal skin infections. Vitamins are fine, but if your dog is on a high-quality dog food, they probably aren't needed. Omega-3 fatty acids can be helpful to improve skin and fur coat, however.

There is also a new product out called Promeris that has been shown to be effective against Demodex. It does contain amitraz, so if you have any cats or small children in the household, I wouldn't recommend using it.

He weights 35 pounds how much Ivomec should I give him orally daily? What antibiotics do you recommend? Thank you,

If you are using Ivermectin at 10 mg/ml (a 1% solution), then you should be giving up to 0.9 ml once a day. If you haven't been giving it daily, then start low (0.16 ml) on the first day, then 0.3 ml the second day, 0.6 the third day, the 0.9 the fourth day, making sure your dog isn't having any side effects (generally neurologic problems). Then stick with the 0.9 ml once a day for months.

Antibiotics you will need to get from your veterinarian. I generally use cephalexin for skin infections.

I checked out the website you mentioned and have to say I'm a little horrified by some of the information there. The disease descriptions were good, but some of the treatments are not. There is absolutely no reason to use Prolate on a dog with Demodex and I wouldn't use it for a dog with Sarcoptic mange, either. The dosages mentioned there aren't always very accurate for many of the drugs and I would worry about overdosing dogs with what is there. His estimates for vet visits is also off-base and you really shouldn't bypass your highly educated vet, even for routine stuff. Trust me--we have had at least 6-8 years of college and vet school, along with about 20 hours of continuing education every year, something your average website doesn't have. So always take things with a grain of salt.

I also wouldn't combine a lot of different treatments. The ivermectin alone, at the dose I gave you, should clear up demodectic mange. The more treatments you use together the more likely your dog will be to develop some bad reaction.

As far as the Fish Flex cephalexin, I wouldn't use it orally in dogs. It does sound like it is the antibiotic, but who knows if it is a good medical grade to give your dog. I suppose you can chance it, but if something goes wrong, it may end up costing more in the long run. Cephalexin isn't that expensive and might be one of those $4 meds at Walmart or Target, so you could ask your vet for a written prescription.

Finally, since the bumps are new, it would be a good idea to have your vet check them out to make sure it isn't something besides a skin infection--like puppy strangles or even a reaction to one of the treatments you've been using. That would require different treatment.

So, Ivermectin as directed, Malaseb baths, Goodwinoil and Cephalexin ( what way given to the Dog, dose for 35lb Dog ) and a strict regimen for a couple of months should do it? Besides the demo, he is doing super ok, eating, playing. Thank you in advanced for all the information.

If you are doing the ivermectin, I wouldn't do the Goodwinol. But if there are only a couple spots, then I'd probably go with the Goodwinol each day and skip the ivermecting. 500 mg of cephalexin every 12 hours for 3 weeks for a skin infection. Malaseb twice a week with a 15 min contact time before rinsing off.
