Question

Is there something I can do to help heal a hematoma on the calf of my leg caused by a car accident? I have read that Rice is good and how would I use it?

I have put ice packs on it heating pad, elevation and went to Dr. and he said it would be ok just take some time. I can hardly walk on my leg and if I am up on it very long it really hurts.

Hello; Thank you for writing in today. RICE is an effective way to speed the resolution of a hematoma. Rest, ice, compression, and elevation are the most commonly prescribed methods when a person has a hematoma. When you use ice, I would recommend using an ice pack for 20 minutes several times a day or a minimum of 4 times per day. Unfortunately, hematomas take time to resolve. It takes the body time to reabsorb the blood. Since you are having a great deal of discomfort, you may want to talk to your provider about using an anti-inflammatory medication, such as Ibuprofen, Naproxen, or Lodine. Generally, these types of medications taken on a more routine basis during the acute phase of pain can really help patients. For some patients, particularly those with renal, gastric, or blood pressure issues, anti-inflammatory medications are contraindicated. At this point, I think it would be wise to talk with your provider about some medication to help manage pain, particularly since your pain is interfering with ambulation. It may take the hematoma weeks to fully resolve; however, hopefully, your pain will subside soon. I hope this helps answer your question. RICE stands for rest, ice, elevation, and compression. It sounds like you are already incorporating most of these measures. With regard to rest and elevation, the leg should be rested and elevated as much as possible. As a baseline, the leg should be rested and elevated at least every hour if possible. As for the ice, 20 minutes is sufficient. Beyond 20 minutes, the body adapts to the ice and therapeutic effect is diminished; therefore, it is best to apply and ice pack for 20 minutes several times per day. You are currently alternating heat and ice. This is fine. Some providers advocate this method. If you find heat and ice beneficial, you can continue with this, but again 20 minutes for the heat too. As for the compression, some patients find wrapping an ACE bandage around the extremity does improve their pain. Added support sometimes helps. I hope this helps.
