Dear Doctor, I hope you can help as Candida is ruining my life. First of all I would like to apologize for my English, I am Italian, so it won’t be perfect! Before to get to the point I would like to write you my history, I hope it can help ito make a diagnosis. I am an healthy, professional, 29 years old female. I got y phd 2 years ago and now I work in London as a banker. The only medical problem (before Candida) is that for all my life I had suffered by frequent tonsillitis (they last just 3 to 4 days but the fever go up to 40*). I get tonsillitis even when I am particular under stress. I say this because I got a lot of antibiotics during my all life time. I met my partner 7 month ago, last June we had sex for the first time. I started the pill for the first time in my life in mid July ( the doctor gave me Cerazzette desogestrel 75 micrograms) but because of the very heavy weight gain I had to stop it after 10 days. At the end of July I discovered to be pregnant, and for the stress I get a very strong tonsillitis again. Luckily it last 2/3 days and I treat it with a week of antibiotics. End of July/early August during a normal visit, the gynecologist confirmed the early pregnancy and said that I had Candida Vaginitis (my partner got it too). The gynecologist could see I had candida but as for now at the first microscopy examination the test was negative, it result positive by growing the Candida on colture. I think it was the first time for me, but because I do not get any symptoms (rather then a normal white discharge) I can’t be sure if I had it before. Anyway, because of the termination ( I took the abortion pill/vaginal tables) I couldn’t treat the Candida for more than a month. Additionally because of the tonsillitis first and termination after in august I took 2 weeks antibiotics and I started to treat the Candida just at the very end of the months (I went to another gynecologist that gave me an oral treatment for 2 days and vaginal ovules for 1 week). Candida disappear and the strange thing is that my partner, even if we made love in august while I was still infected didn’t not get it (he treat it in July with a local cream for one week only and got the same treatment as me in the beginning of September even if he didn’t have any symptoms). 11 of September I started again the pill. This time the doctor gave me Lybella (apparently a very light new pill, similar to Yasmin) Now, because my partner got the same symptoms he had in July I went to the gynecologist again, following up the details: 2)Saturday 24th /10: a) Gynecology visit - candida was negative on first microscopy examination , but positive result by growing the example on culture (got the test result on Thursday the 30th) b) I get 150 mg of FLUCONAZOLE 3) SUNDAY 25th /10 : I get Clotrimazole Pessary 500mg 4) Wednesday 24th: start the proboscis, vitamins and propolis cycle (to end in 6 weeks) 5) Thursday 29th/10 a) Candida came back b) Gynecology visit. Again, negative on first microscopy examination, but positive result by growing the Candida on colture (I did the test on the 24). c) I get 2 capsules of SPORANOX 100 mg (at 20:30) d) I start the cream (to end in 7 days) 6) Friday 30th/10: I get another 2 capsules of SPORANOX(itraconazole) 100 mg (at 08:00) Candida seems to improve but doesn’t disappear ( still have a bit of white discharge) 7) Monday 1st/11: I got my period, so can’t see if I have Candida or not. I would like to underlain that my partner had an HVI test and he is negative, as well as I have no allergies problem, no Diabetes or any other illness. My blood result (I did it 2 weeks ago) where perfect. Last Friday I finished my second month of the pill… I am starting to get really worried and I would really appreciate if you could kindly give me your opinion.

Thank you for your history - This seems like you have a very long history of uncontrolled or repeatable yeast infections - is this correct? There is a way to treat this.

Hi, as I said that I am aware of I had Candida just twice. First time in the end of July (who I kept for one month as I couldn't treat it because of the termination) and now.

I do not know if I got it before these 2 episodes, as I always had a bit of white discharge but never any other symptoms. Surely I didn't have on the 10 of July because I went for a normal visit and the test was negative.

Why do you think this time my Candida doesn't respond to treatment?

Hello,
There are many different subspecies of Candida - the most common is Candida albicans, but another one called Candida glabrata does not respond well to routine treatments and only works with extended treatments with either Terconazole or Diflucan. If you use Terconazole you might need to use the vaginal suppositories for a 2 to 4 week period, and with the Diflucan you might need to take the pill every other day for about 2 weeks then once a week for the next 2 months.

Dear doctor thank s a lot for this so do you think it is another type of Candida?

doesn't not mean that my immune system is weak? My concern is, could the fact that my Candida doesn't respond to treatment, or the fact that I get another kind of Candida (less common) be a symptom of something worst?

Yes, usually when I do another culture this what I find - I usually treat with either of the above medications and my patients to very well. Your immune system is probably fine - but every once in awhile it is okay to have a yeast infection - it could be due to stress, anxiety, get sick, or even your diet. I tell my patients to eat well, eat plenty of protein, eat yogart and try to stay away from fatty foods as much as possible. The bottom line is that you want to keep the acid-base (the pH) in balance and this will help with future infections. VERY unlikely it is something worse given your age. I will surely click on accept. Can you just tell me, if it is not another type of Candida, if it is just albicans but it doesn't respond to treatment. What do I have to do in this case? and what does it mean? it is true that candida is a early syntomps of HIV? And do u think I could always have had it and never realize?

 

P.S. last stupid question, during my period does Candida goes away.. or I just can't see it?

This could be dysplasia or a contact dermatitis - but I do not think so. Candida is NOT usually any sign of HIV - but if concerned, get an HIV test - I usually test my patients for HIV every other year, just as a safeguard. Take care, thank you in advance for the accept.
