Question

On October 12 I woke up with extremely swollen lymph nodes and a sore ear and throat, all restricted to my left side. A throat culture revealed no strep, and the lymph swelling went away within a few days. I was prescribed a steroid nasal spray which I used for a week and told that I was just having bad post nasal drip. I have also been doing nasal rinses with a neti pot and taking decongestants. However, all this time later I am still congested, my left tonsil is still visibly swollen and I am snoring like a maniac at night. (I don't usually snore.) The post nasal drip is still an issue, though the mucus is clear and I don't have very much facial pressure. I know colds can linger, but this is getting ridiculous. I'm not breathing well and not sleeping well because of the snoring. I would love to know what could be causing these extended symptoms, and if I should continue with some kind of at-home care or see my doctor.

Since your mucus is clear, I am doubting you have a raging bacterial infection (including strept). However there are other factors that can be causing such a long term problem. The first is a possible viral illness (a cold or flu), but you are correct that this is way too long to be typical of any cold or even the flu. The second would be allergies. Prior to October 12th, did anything change in your household? The laundry soap? A new pet? A new deordarant or perfume? And the third would be a more serious problem like lymphoma, but before you get upset, this would be rare and most likely not the problem, but I need to make you aware.

More than likely it is one of the first 2 I listed or a combination of both. If you have other persistant symptoms like night sweats, prolonged fever (greater than 5-7 days), poor eating, then I would be more concerned about something more serious. In that case you should be seen by your doctor ASAP. Please let me know about any other symptoms, and any new things at home and we can go from there.

Hi Nurse Tracy,

I think it's possible that I have seasonal allergies, since I have the tendency to get sick around the end of September/beginning of October, although at that time I usually get very obvious symptoms of a sinus infection which clear up with antibiotics. I have had my dog for several years, and I actually make my own laundry detergent (last batch was made over a month ago) and don't wear perfume, so no changes there. I also don't seem to have a fever. I do sometimes get a little sweaty at night.

My appetite has been good, but I have had a dull ache in the lower left side of my abdomen for the last several days that isn't relieved by bowel movements and isn't likely related to menstruation - my "period" ended a week ago and I'm on the pill. I din't mention it because I didn't want to bring up too many issues at once, but perhaps it is relevant? Please let me know if you need any more info.

I do not think the birth control pills are an issue (unless you smoke). The dull ache in your left side could be referred pain from your spleen (but without an examination, there is no way to know for sure) or possibly even pain from your kidney. However your spleen and lymph nodes are well connected so it only makes sense that if you have swollen lymph nodes for a long period of time, your spleen would be directly impacted. Having no fever does not necessarily rule out an infection (it is rare, but some people can have an infection and not run a fever). At this point, you have had swollen lymph nodes for well over 2 weeks with the other symptoms....I do not like to just point people to their doctor; however in cases such as yours, there really is not much anyone else can help with without a physical exam and running blood tests. You need to have your doctor rule out mono, a sinus infection that is not presenting as it normally does and other things. It is still possible that it is just a bad year for allergies for you. I noticed you have taken decongestants, but have you tried an allergy medication like Claritin yet? You may want to try it for 2 days to see if that does not help alleviate some or all of your symptoms first.
