Question
our maltese has medial patella luxation. He squats and shivers up to three times per day. It is getting worse. Is surgery the only answer. he has been on a non-steroid anti-inflamatory. When his events occur, I caress him lift him from floor slightly and try to massage from is hips downward to re-assert the patella. sometimes with success sometimes without.

While I cannot diagnose this over the internet I can tell you the following :
With this condition you only have two real options, one being rest plus pain relief such as Metacam when there is an episode. The other option as you are aware is surgery.
Quite frankly if the conservative approach is not working and your dog is in frequent pain then the way to is surgery, you have no other options.
Sorry I cannot suggest any other alternatives.
