Question

My 14 yr old cat has had diarrhea since July. It is always watery (like soup) and always tan(slightly yellowish cast);sometimes the diarrhea contains very tiny, formed blackish stool in it, sometimes it's just watery, and sometimes it contains red,jelly-like matter. He vomits occasionally. The diarrhea is occasionally extremely foul-smelling, other times it doesn't smell that bad. Upwards of $500 has been spent on testing and meds so far yet nothing has changed. This go around the vet has him on FortiFlora (sprinkled on food) and he was switched to dry Science Diet Sensitive Stomach weeks ago (he wouldn't eat the Rx ID or RD). He is strictly a house cat. There has to be something else that can be done to get to the bottom of this. The vet is putting off the invasive procedures--all the while I worry that something very, very serious is wrong. Any suggestions?

Blood and fecal tests have been done several times, a complete blood cell count, and an X-ray. The x-ray showed a lot of gas in there and he was diagnosed at that time with gastroentiritis. As for the other tests, I was told that his liver and kidneys checked out okay and that he did not have a thyroid problem (can't remember if it was hypo-or hyper- the vet checked for). Could he have FIP by any chance? I ask because I fostered a mama cat & her 5 kittens in July (found on side of road) for a week until a local shelter was able to make room for them. About a month later this shelter had to euthanize 19 cats because of FIP & it wasn't determined which cat the FIP originated with.

Not all of them, just the ones I mentioned in my previous email. I just found one of my vet bills from 9/19/09 and it includes: Exam, Fecal smear, Metronidazole 250 mg, Flagyl, Endosorb Anti Diarrheal tablets, and thyroid profile. I'll keep looking to see if I can find some of the other bills.

Thanks for the information. Chronic diarrhea can definitely be a difficult problem to find a cause for. Luckily, there are still some tests that can be ran for a diagnosis. My biggest concern is the blackish color in the stool and the red-jelly like substance. Usually, blackish color in the stool is digested blood. The reddish color may be fresher blood. I have a suspicion that something in his stomach or intestinal tract is causing bleeding. Possibilities include gastrointestinal ulcer, intestinal parasites (roundworms, hookworms, Giardia, Cryptosporidium, abherrant heartworms), chronic pancreatitis, bacterial infection (Salmonella, Campylobacter, Yersinia, Clostridium), fungal infection (Histoplasmosis, mycobacteriosis, phycomycosis), cancer and an immune mediated problem (inflammatory bowel disease). Routine bloodwork may or may not pick up bleeding in the gastrointestinal tract (usually the BUN is elevated, but not always). I know multiple fecal exams were negative, but I never let any of these guys leave the hospital without trying a therapeutic round of Panacur. Some parasites are more difficult to pick up on a fecal exam than others. Panacur is probably the most all inclusive dewormer. I definitely use metronidazole for the anti-inflammatory effects. I would probably add Clavamox to the regimen for more antibacterial control. An H2 blocker like Pepcid may help to protect the intestinal tract better if dealing with an ulceration. Sucralfate would be a good medication to use to protect and heal the stomach lining. These are all medications I would consider for empiric therapy. As far as getting a diagnosis, I would start with a fecal occult blood test to check for the presence of blood, stained fecal cytology (you would have to ask your vet if this was included in the fecal exams), Giardia SNAP test, a fecal bacterial culture, test for feline leukemia and feline FIV (if not already done), Feline PLI (pancreatitis) and an ultrasound of the abdomen. Endoscopy of the stomach and intestinal tract may be your best bet for getting a diagnosis. From here, a biopsy of the intestinal tract can be taken without major abdominal surgery.
