Question

Hello, I am currently in Cambodia and 2 days ago developed what I think is a yeast infection. I have used Gynecon for the past 2 days and Daktacort during the day but my symptoms are not getting better, worse actually. Am a little desperate to sort this out quickly as I fly back to the States in 2 days and the prospect of having to face an already challenging 24 hour journey with intense itching, abdominal pain and unbearable discomfort is just unthinkable. As I said I'm desperate, I currently have at home an athlete's foot cream that I don't think is meant to be used internally, but am willing to try if it will help. I also have a natural nail fungus ointment, the ingredients are Neem Seed, Tea Tree Oil and Lemon Bioflavanoids. Should I try either of these internally? Or is there something else I can try to find to find here that will work immediately to avoid a hellish full day of international travel? (Please know that Cambodian health services are lacking to say the least and the pharmacies may not have new medications you would find in the States). If anyone could offer a few different fast-acting options I would be extremely grateful.

Greetings and welcome to justanswer,

 

There are some common causes of such itching and abdomen pain.The common ones are bacterial or fungal infection, stress, vaginal atrophy, hormonal deficiency, menopause, etc.

 

The infectious cause should be ruled out first as per the symptoms described by you.It may be caused by bacterial or fungal infection.The bacterial infection are treated with antibiotics like metronidazole, ofloxacin, doxycycline, etc. The antifungal drugs like miconazole, ketoconazole in the form of vaginal creams or tablets are used to treat the fungal cause of such symptoms.

 

You are already taking the antifungal drugs and may add the antibiotics like tinidazole cream to your current prescription.The relevant investigations like pap smear, blood counts, are also advised to know the associated abnormality and cause of such symptoms.The pain may be releived by the use of analgesic drugs like motrin , advil which are available as OTc drug.

What oral antifungal tablets should I try to relieve this quickly? I also understand that while antibiotics would help if this is a bacterial infection, isn't it possible that they could aggravate things since many women develop yeast infections while taking antibiotics? If there is no chance it would make things worse I am willing to try an antibiotic, could you suggest a particular one and should it be an oral or topical treatment? And would you reccommend I try internal application of either the natural fungus ointment or the athlete's foot cream I mentioned before?

You may try any one of the available oral antifungal if you want.The vaginal antifungal cream like ketoconazole, fluconazole are also very effective in controlling the infection.

The bacterial infection should also be covered as they may also cause such symptoms.The oral antibiotics commonly used are metronidazole, doxycycline.You may try the oral metronidazole drug tablet available on prescription.

The vaginal cream should be used for internal application and other options should not be used inetrnally in vagina.

Okay, so there is no chance that antibiotics would make the symptoms worse? If I have to sit on a plane for 20 hours feeling like this I will go crazy.

You may take the antibiotics as it will definitely help you in your symptoms related to bacterial cause.The avoidance of antibiotics in case of bacterial infection may more worsen the symptoms.

But in the case it is not bacterial and is a fungal infection, would the antibiotics make it worse because they would kill the good bacteria fighting the fungus?

Its a good question and that is the reason that the pap smear, vaginal smear and other investigations are done to know the causative organism.The treatment is according to the cause.

Since you are having the symptoms even after the use of antifungal drugs so the chances of bacterial infection might be there and should be treated and more specifically your condition should be treated after investigations.

You may take the drugs as it will ease your symptoms of itching and should not worry about the good bacteria as they will get replenished again after some days.

Unfortunately I don't have the time or access the resources for any tests or examination to determine the exact cause prior to my journey. I am just trying to get back to the States in a state of relative comfort, if the problem persists I can seek further treatment there. I will try the antifungal treatments again tonite and if there is no improvement I will get some antibiotics so hopefully I will not have to endure this during my very long journey starting Mon. morning. I appreciate your help and would also appreciate it if you would let me know if there are any more suggestions you can think of to alleviate this problem before Mon. If the antibiotics can ease the itching and pain I would be ecstatic. If you wouldn't mind also answering one last question, would you suggest the antibiotic in an oral or topical form for best effect? Thank you for your help.

You may take the pain killers like motrin or advil for the control of pain symptoms.The application of steroid-antifungal cream may help you in itching.

The oral metronidazole is a better alternative as it penetrates in the deep organs as well. You may take it in the dose of 2 grams in a single dose.
