Question

my friend has experienced some symptoms and I am wondering if he should see a doctor right away. they started about 30 minutes to 45 minutes ago. He first told me he was experiencing tingling and numbness. Then he was talking very slurred or something similar. Then I noticed his fingers were contracted and his arms. I stopped the car and he got out and was on his knees. I dont' know it could have been a stroke or a heart attack or a seizure. He drank alcohol for the past 5-7 days and stopped drinking this morning--so I also don't know if it could just be alcohol detoxification? the symptoms started a bit slow--got severe-- and then went away kind of slow. Now he seems to be ok, but he does still have a tightness in his chest. I had him smile and his smile is even. I had him lift his arms and they are even. He has no insurance and doesn't want to go to the doctor. Do you think he is ok now that the symptoms have dissapated-- or will they perhaps come back--worse.

How old is he? Any history of heart disease, high cholesterol, diabetes, high blood pressure? Is he obese, sedentary, stressed, smoker?

he is 42. no history of heart disease, nor high cholesterol (he hasn't checked it for a few years though), no history of diabetes. He is not obese. he weights 185 pounds 6'1 tall. somewhat sedentary, but he walks generally everyday about 8 miles. He smoked for 20 years, but quit about one year ago. He has been quite stressed and has a history of alcohol abuse. He did quit drinking about a year ago, but has relapsed about once every 5 months (he will drink for about 5 days--and then suffer for about 5 days) Also he drank about 2.5 gallons of voldka over this past week and hasn't eaten much. Today he did have a few bites of food--and he had some milk thistle and activated charcol

Hello,

 

It can be transient ischemic attack (TIA). This is just a small duration decreased blood supply to the brain. This is a warning sign to look for the atherosclerosis or narrowing of the neck arteries (carotids), which supply blood to the brain. The alcohol withdrawal or detoxification is not for short duration and persists longer. With his medical history, one needs to keep an high index of suspicion. If he is absolutely normal he may consider going to a walk in clinic (urgent care) where vital parameters can be taken and EKG can be recorded. He can get the rest of the investigations and medical imaging later on in a routine appointment. That might be less expensive alternative to ER where they may decide to all investigation including echocardiography, MRI or CT scan, blood work etc.

Thank you. Also, I forgot to mention that he is having a strange pain in his chest. He threw up all last night, and had hiccups for the past two days as well as throwing up off and on for two days. now when he swallows it really hurts. He says it feels like when you accidentally breath in your food and it gets stuck. Looks very painful. Is this something that he should worry about enough to see a doctor in an emergency room? or can this wait as well? thank you again!

These symptoms would warrant to take him to ER, as they are slightly more serious and need a chest x-ray and full cardiology evaluation, I am afraid. A Mallory Weiss syndrome should be ruled out. Please read about it;

ok, I read the link about mallory weiss symptom, and it said the symptoms include bloody stools or blood in the vomit. It also said that the tear usually heals in about 10 days without treatment. So I am wondering why the ER? Can you explain?

 

Also, what do you think caused the sudden paralysis, numbness, and slurred speech?

Hello,

 

The Mallory Weiss may not have blood in vomiting or stool when the laceration is very small. Once it is suspected, it needs thorough evaluation as it may heal spontaneously or tear may get bigger and cause complication. So both the spectrum of prognosis are possible. His symptoms can be due to;

 

1) TIA; a decreased blood supply to brain tissue

2) Cardiac problem; a heart attack can decrease the blood flow to each and every part of the body.

3) A sudden chest pain unrelated to heart (termed non cardiac) causing fall in blood pressure due to the intense pain and resultant fall in blood pressure and thus decrease blood flow to the brain and other organs.

4) Unrelated to above; some other phenomenon which we may not be able to understand due to lack of physical exam.

 

The crux is; with his symptoms he needs an examination by a physician. You may consult at urgent care clinic and if the physician decides to send you to ER, for specific reason, you may consider that just to cut the expense.
