Question

I am under treatment for Gerd and panic attacks. I also have neck and back issues. I was in to see a doctor two days ago and he changed the lorazapam to clonazapam. I have been under great stress as I am the primary care giver to my parents. My father has congestive heart failure and CLL and my mother is a blind diabetic with stage 4 renal failure. During the day I seem to be fine but for the last week or so I get sick to my stomach, jitters and a burning sensation like a warm felling in my body, chest and arms after I lay down to sleep. should I have any concern that its a heart issue. I have had cardiac evaluations in the past which were all fine and defined the panic disorder. I am just concerened with the stress that these symptoms could be cardiac in nature. I have taken my pulse just now and it is 54 and my BP is 116/84. Is it reasonable to think it is just the stress/anxiety bringing on more Gerd

Answer

Hello,
As your previous cardiac investigations were fine it is less likely to have any cardiac pathology.
There is a connection between stress and GERD.
When your are under stress you tend to eat more which aggravate your GERD symptoms .

To relieve stress and symptoms of GERD, you should do exercises (walking for 20 minutes daily ), medication , yoga , deep breathing exercises .
You should eat specific diet for GERD.

Even you need cognitive behavioural therapy , psychotherapy to relieve stress .
I suggest you to consult a psychologist for further evaluation and management.
I hope this helps.

I have not eaten much of anything this week because i was under so much stress and at the hospital with my father, yet I still feel like I need to vomit and have, after I lay down. I recognize the need to exercise and need to find the time to do so. I am under the care of a therapist. My concern was the link between the stress and any cardiac issue. A secondary question would be how recently would the cardiac investigation need to be to rule it out as a concern? The cardiologist did say that things were fine and there wasn't a need for follow up at the time, that has been 2 years ago,(at that time i did a chemical stress, blood work, echo and ct, again all fine) I have been in to see my physician several times since then without concern for cardiac issues in fact she lowered my BP med and said i could come off completely if i wanted. I quess i was looking for reassurance that this was just Gerd/ stress.

Hello,
As such nausea , heartburn, burning sensation in chest , stomach are all suggestive of GERD.
Panic attack are very sudden, discrete periods of intense anxiety and stomach problems and discomfort, expereincing pain like heart attack which is most likely in your case.
Any cardiac problem is less likely in your case so don't panic at all.
Don't skip your meals and eat frequently small meals .
Drink plenty of water and vitamin rich juices.
If your symptoms 'll persist even after taking your prescribed medications then you should be seen by your doctor and undergo an EKG as a precautionary measure.
Apart from this as your BP is not high so discuss it with your doctor to adjust your dose or taper it gradually.
I hope this helps.
