Question

My 6 year old cat has lost a significant amount of weight in the last couple months, has been vomiting frequently and feels warm to the touch yet he has been eating and acting normally and does not have a fever. The first visit to the vet showed his liver enzymes were elevated AST 173, total protein 8.9, WBC 30.4, neutrophils 17024, lymphocytes 11856 and bilirubin 0.4. He has been on a course of clavamox for the last 3 weeks as well as actigal and denamarin. I also gave him a course of droncit in case he had liver flukes. His blood work was redrawn a month after the first draw and his AST was 174, total protein 8.3, WBC 25.0, neutrophils 13000, lymphocytes 11000 and bilirubin 0.4. He has remained afebrile but continues to feel warm to the touch but is eating well acting normal. I think the vomiting has decreased but still continues occasionally. Any idea what could be causing this? I am not sure what to what to do next. Ultrasound, liver biopsy and what. Thank you XXX XXXX

No, he has actually gained 10 ounces in the last month (since his first visit) and does not appear to be drinking an excessive amount of water (no more than normal).

Long ago when I first got him neutered I had him tested. He has remained an indoor cat aside from the occasional slip out the door.

Hi,

 

The first thing I would recommend having done is a bile acid assay. This is a blood test that will determine if there is actually a problem with the liver or not. The AST is not an organ specific test. It can indicate liver disease, but that isn't always the case. Stress can cause it to be elevated among other things. If the bile acid assay is normal then I would not do the liver biopsy. An ultrasound would probably be the next step.

 

Elevated neutrophils almost always indicate bacterial infection, while your cat's aren't super high (normal is up to 12,000) it is definitely elevated. You may want to consult your vet about trying a different antibiotic. Its possible that the bacteria isn't susceptible to clavamox.

 

It is a good sign that your cat has gained weight. It makes me a little less inclined to think there is something really bad wrong. Generally, the cat will simply continue to deteriorate if this is the case.

 

If your vet can't find anything wrong with further tests I would continue to monitor his weight, and re-check the blood tests again in another month or so as long as he continues to eat and act okay. If he should happen to worsen then you may want to consider taking him to a teaching hospital where there are multiple specialists and all the latest diagnostic equipment.
