Most probably it may be a bite from a insect or an ant.By looking at the description it looks that there is severe inflammation which may be infected if not taken care properly.You should do following to have relief-

1.Do ice fomentation to reduce the swelling.You should know if the swelling does increases & includes the large part of the forearm then it may may compromise the circulation of the forearm as occurs in the compartment syndrome.

2.Give him an anti inflammatory analgesic like advil.

3.He should have a broad spectrum antibiotic like ciprofloxacin to prevent infection in that area.

4.If swelling is severe enough then you should consult to your doctor,he may prescribe a steroid to reduce the swelling.
