An ultrasound is essential to look for pelvic inflammatory disease. This can cause a vaginal discharge with the smell. Metronidazole should be tried to see the therapeutic response as this is specific for anaerobic bacteria and other antibiotics may not work on these bacteria. Since the swab has not shown anything positive, special culture for the yeast and mycobacterium should be done for the discharge. This should hopefully clinch the diagnosis.

Smell as a rule points towards the infection. The cause and source of the infection needs to be found by the investigative techniques. As I mentioned some atypical infection needs to be thought of and medical imaging like ultrasound, CT scan or laparoscopy has to be done for further work up to clinch the diagnosis.
