If she is having periods, then pregnancy is unlikely.
If you suspect that she may not be practicing safe sex, it would be beneficial to take her to her physician to have her tested for sexually transmitted diseases such as gonorrhea/chlamydia.
You may want to consider a discussion about starting birth control pills as well. This would also help her if her periods are painful.

Her breast enlargement and itching may be due to hormonal enlargement/development. Again, pregnancy is unlikely if she is having monthly periods but should be ruled out by her doctor. Especially if she is having enlargement and hardening of the area below her navel.

So to answer your original question--Yes, I do think she needs to be checked out by her doctor. She needs blood work and an exam.
