Question

I have a male cat, age 5, just had blood work with high white blood count, high nutriphils and lymphocytes, has loss of weight, puffy and peeling front pads. My vet wants to give him 3 days of fluids and put him on antibiotics. He suspects leukemia, kidney infection, lupus, or maybe cancer. I am almost sure he was vaccinated for feline leukemia. "Buttons" is eating good, no vomiting or diahrrea, playing and acting normal. He lost only 2 pounds in 6 months, but I can feel his spine. He has also had problems with his colon for 2 years (chronic megacolon). What is your opinion?

Although we do not see this often we have encoutered this a few times

I would ask your vet if he thinks this may be Plasma Cell Pododermatitis.
This is an uncommon condition that affects all or just some of the foot pads in cats. In my experience it comes on quite quickly without any known cause. The pads begin to swell and get puffy, eventually peeling. Often the cats show no pain, discomfort or sickness, this can often be accompanied by a secondary infection which would explain the elevated white cell count. We treat this with antibiotic called doxycycline. We use this drug because of both its antibiotic properties and its immuno-modulating properties as this condition is thought to be immune mediated.
