Question

I am just starting on Zoloft and have been having bad side effects. Initial dosage was 50 mg and after 4 days had panic attack with rapid heart rate. I have also been experiencing tingling, worse night sweats (perimenopausal and have been having some night sweats), and hot flashes. After my panic attack my Dr wanted me to half my dose for the next week (25 mg) and then try the full dose again. I have not had another panic attack but still have the other side effects and lower level anxiety so I have been taking .25 mg Xanax which relieves the anxiety. I have been on a half dose for two days. My question is how long should I give the Zoloft? Do the side effects go away over time?

Hello Mam

Zoloft (Sertraline) is a drug which belongs to SSRIs class of antidepressants. All of the SSRIs including Zoloft can initially cause anxiety in patients taking it which is transient and goes away with time. For this reason, initially Benzodiazepines such as Xanax are prescribed with SSRIs in patients who have anxiety component also to control the anxiety symptoms.

As clinically seen Zoloft is very well tolerated by most of the patients and the side effects if develop go away slowly within a week or two.

The starting dose of Zoloft is from 50 mg per day in patients of depression and OCD, and if patient is having Panic disorder the starting dose is 25 mg per day which is slowly increase and to 50 mg per day after 1 week thereafter. Maximum dose is usually 200 mg per day.

The dose of Xanax can be increase in your case as you are using a very low dose after consulting your doctor .
