can't locate our CP575 for issued from the IRS
we are run by the town the health center is located in
I have the TIN but no verification in writing from the IRS, we use a direct deposit process thru quickbooks for
payroll taxes so therefore a quarterly tax coupon isn't
available either. help

not that i'm avare of... the only thing i have recieved is that this letter is needed in order

to process my submittedd IAC"S request from CMS's ezxternal user services support center....sorry

Hello again CVsusan,

The only way to obtain IRS Letter 147C is by sending a written request to the IRS. This information cannot be requested on line or over the phone.

You must write to the service center where you file your tax return, Attn: Entity Section, and ask for IRS letter 147C, "EIN Previously Assigned" . If you have it, you must enlcose a copy of any "B" notice you received.

If you do not have a "B" notice, then include a cover letter stating the circumstances of why you need this letter.
