Question

My 88 year old father is planning to hire an RN or LPN to live in his home to help care for his 85 year old wife with Alzheimers. The kind of care she needs is mostly personal cares, and they need homemaker services such as cleaning, grocery shopping, meal preparation, companion care while he is away etc. As far as "skilled nursing services needed" at this time it is minimal. She may need medication monitoring at the most but no other services that would require a liscensed nurse. My dad thinks he needs to hire an RN or LPN to get a full tax write off of somekind. His tax accountant has told him something like this which he has interpreted to mean that it cannot be a home health aide or PCA but an RN or LPN and the entire cost could be deducted or written off on his taxes. Is this true even if it is not skilled nursing care?

Please give as many details as to what kinds of health care services are either tax deductible or a write off when looking at hiring 24 hours home care. They will be hiring privately and not through an agency (too costly) since the patient is needing supervision, personal cares, household help, cooking, laundry etc. plus med management does this qualify for any tax deduction? Does it need to be an RN or LPN to get the tax benefit or could it be a Home Health Aid or a Personal Care Attendant. this will be a "private pay arrangement" Their income is mostly social security plus an annuity and perhaps some other interst bearing accounts but their annual income stricly is very moderate and they are looking for any tax advantage they can find to hire homecare. Assuming - we are talking about medical expenses deduction - let's see for reference IRS publication 502 - http://www.irs.gov/pub/irs-pdf/p502.pdf

Nursing Services
You can include in medical expenses wages and other amounts you pay for nursing services. The services need not be performed by a nurse as long as the services are of a kind generally performed by a nurse. This includes services connected with caring for the patient's condition, such as giving medication or changing dressings, as well as bathing and grooming the patient. These services can be provided in your home or another care facility.
Generally, only the amount spent for nursing services is a medical expense. If the attendant also provides personal and household services, amounts paid to the attendant must be divided between the time spent performing household and personal services and the time spent for nursing services.
You can also include in medical expenses part of the amount you pay for that attendant's meals. Divide the food expense among the household members to find the cost of the attendant's food. Then divide that cost in the same manner as in the preceding paragraph. If you had to pay additional amounts for household upkeep because of the attendant, you can include the extra amounts with your medical expenses. This includes extra rent or utilities you pay because you moved to a larger apartment to provide space for the attendant.
Employment taxes. You can include as a medical expense social security tax, FUTA, Medicare tax, and state employment taxes you pay for an attendant who provides medical care
