Question

Hi, our 13 yr-old pug got kennel cough despite a previous vaccination. His coughing has persisted since June 2009, with some improvement at times but it has never completely gone away. He is currently coughing after any activity or excitement. Our vet here in Switzerland has prescribed various antibiotics, low-dose prednisone (1 mg / day), anti-cough tablets, and homeopathic cough liquid, as well as given him injections in the office. He is also on Karsivan and Fortekor. Any ideas? Is this going to be a permanent affliction for him? His health, energy, and appetite are otherwise good.

Hi,

This sounds like Tracheal problems such as irritation or a collapse during times of excitement or exercise. This is a common condition in pugs, and often a previous medical condition such as Kennel Cough or a Upper Respiratory Illness can bring this on. I suspect that it is not the kennel cough that is persistent but instead tracheal collapse which has stemmed from the original infection.

Tracheal collapse is a condition in which the trachea partially collapses as a dog breathes, leading to tracheal irritation, coughing and sometimes secondary changes in the lungs or upper airways.

The irritation is self perpetuating, since coughing and increased respiratory efforts lead to further irritation and worsening of clinical signs. Eventually, damage can occur to the lungs, larynx or even upper airways. Obesity, irritants, allergies, concurrent heart failure, bacteria, viruses can all contribute to the problem, as well.

The symptoms of tracheal collapse are coughing, difficulty breathing and tiring easily. When this condition first occurs many dog owners truly believe that there must be something caught in their dog's airway due to the severity of the cough. If the tracheal collapse is occurring inside the chest the increase in pressure on the circulation can lead to heart enlargement or contribute to heart failure.

There are medical treatments for collapsing trachea as well as surgical treatments. The consensus of opinion seems to be that collapsing tracheal problems are best treated medically until it becomes obvious that medical treatment alone is not going to work.

 

What I would recommend is you set up a appointment with your vet to have the tracheal examined to determine how bad it is and how they want to proceed with treatment.

Dear Walter,

Thank you for your answer. Tracheal collapse is definitely something I will talk to my vet about when we go this week. Just one question before I click "accept" -- is a collapsing tracheal problem something that would have shown up on chest x-rays? Because our pug has had two sets of those, I believe, and the vet did not see anything of concern.

No a chest xray would not show this, he will need a scope to get a definite idea wither he has a collapsed trachea or not.
